Photo copy of driver license
Photo copy of social security card or passport or birth certificate
Photo copy of auto insurance (100/300/100 coverage limits)
Current Resume
Passport style photo of you in a jpeg format- do not stack this in PDF, send as separate attachment
Federal Background Investigation – U.S Staffing procedure is that all potential adjusters that are joining our roster be screened for the following:
- Conviction or pending charges of a Felony, DWI, theft or current criminal
history
Code of Conduct ( included in application)
Confidentiality Agreement ( included in application)
Drug Test Consent Form ( included in application)
Independent Contractor Agreement Experienced Adjuster Form (included in application)
Direct Deposit Form ( included in application)
W-9 form ( included in application)
I-9 form ( included in application)
